人际同步性现象:探索心理咨询过程中同盟关系的新视角

人际同步性是指两个或两个以上的人的言语特点、行为表现、生理指标以及神经生理活动在时间上出现重合的现象, 得益于近10年内技术上的突破, 研究者开始对心理咨询领域的人际同步现象进行初步的探索, 试图从该视角揭示心理咨询过程中咨访双方的互动模式和同盟建立的内在机制。近期研究主要关注非言语同步、生理同步、人际脑同步与同盟及其影响因素之间的关系, 并针对它们各自对同盟的作用机制发展出了相应的理论。未来的研究者可以更多地关注同盟动态变化过程中的人际同步性, 发展整合理论来回应同盟建立的内在机制, 并在临床应用上促进具有人际同步性的心理咨询互动过程。

Interpersonal synchrony (IS) is the co-occurrence of speech features, behavior as well as physiological and neurological activities of two individuals or more. Over the past decade, ever-increasing advancement in the measurement methods of this phenomenon has prompted researchers to make a thorough investigation into almost every aspect of interpersonal interactions, an important research field of which is the therapeutic interaction between the counselor and the client in psychological counseling. Clinical psychologists attempt to reveal the interactional patterns between the psychological counselor and the client and to understand the mechanism underlying the successful establishment of working alliance from the perspective of interpersonal synchrony. This, to a large extent, has addressed the predicament in the study of alliance, that is, a lack of research on interactional features between the counselor-client dyad combined with outdated measurement methods solely dependent on self-report data. To elucidate the interactional features of alliance, researchers mainly focus on the relation between non-verbal synchrony, physiological synchrony as well as interpersonal brain synchrony and working alliance together with its influencing factors. They found that physiological synchrony between the psychological counselor and the client indicated more positive working alliance, and might serve as an objective measure of the counselor's degree of empathy; however, the bidirectional relationship between non-verbal synchrony and alliance and the likelihood of non-verbal synchrony acting as a supplementary form of assessment of alliance quality still require further investigation. Besides, interpersonal brain synchrony has the potential to be an additional neurological indicator of the successful formation of alliance. In terms of the mechanism underlying the successful establishment of alliance, extant theories, such as Shared Intentionality Theory, Polyvagal Theory, Social Psychological Theory, Resilience Model and so on, illuminate the rationale behind how interpersonal synchrony brings forth working alliance and even therapeutic effect. Besides, Oxytocin Synchrony Hypothesis focuses on the biological mechanism of the formation of alliance while Interpersonal Synchrony Model (In-Sync Model) combines different modes of interpersonal synchrony (i.e., non-verbal synchrony and interpersonal brain synchrony) and provides a more integrative theory to uncover the mechanism. Nevertheless, there are still some key issues yet to be addressed in this research field. First, whether interpersonal synchrony can act as an objective indicator of the successful formation of alliance requires further evidence. Additionally, while it is vital to differentiate between different components of alliance (i.e., trait-like component and state-like component) and different types of non-verbal synchrony (i.e., counselor-led synchrony and client-led synchrony), extant research seldom takes the dynamic nature of interpersonal synchrony into account and investigates its contribution to the state-like component of alliance. Furthermore, there still lack theories and empirical evidence devoted to integrating different modes of interpersonal synchrony to uncover the interactional features during the formation of alliance, with the majority of current theories only centered on a single mode of interpersonal synchrony; although the In-Sync Model tries to take both non-verbal synchrony and interpersonal brain synchrony into account, it lacks empirical validation and requires further refinements. Future researchers should integrate different types and modes of interpersonal synchrony when studying its relation with alliance and other influencing factors, pay more attention to the dynamics of interpersonal synchrony alongside the change of working alliance, develop more integrative theories to reveal the essence of working alliance, and, in clinical practice, promote the type of interaction in psychological counseling that features interpersonal synchrony between the psychological counselor and the client.

1 引言:心理咨询中的人际互动
现代心理咨询或心理治疗最早叫做“谈话疗法”, 存在大量的谈话或人际交流的部分, 但是, 咨询师和来访者在一定的时空中互动, 不仅通过言语, 也通过身体行为来交流, 言语互动和非言语互动共同组成了咨询师和来访者之间的人际互动。这种发生在咨询室里的人际互动, 与日常交流、母婴互动、亲子互动乃至教学互动相比, 又有其独特之处。具体而言, 心理咨询中的互动是在结构化或是半结构化的咨询过程中进行的, 咨询师和来访者需要通过共同努力减轻来访者的困扰, 而这种咨访双方的共同努力被称为工作同盟(working alliance)、治疗关系(therapeutic relationship)或是治疗性连结(therapeutic bond) (Koole & Tschacher, 2016), 在本文中以下统称为同盟(alliance)。
实际上, 这种发生在咨询师和来访者之间的同盟关系是心理咨询工作的重要基础。早在上世纪60年代, Bordin (1979)就整合了不同的理论, 将同盟定义为治疗过程中咨询师和来访者之间的合作, 包括目标、任务和情感联结三个维度。同盟也是跨越不同流派的心理治疗的核心(Horvath et al., 2011)。在过去的近50年中, 同盟或有效的咨访关系可能是现代心理治疗领域研究最为广泛且重要的主题, 例如, 国内研究者对同盟进行了概念本土化(朱旭, 江光荣, 2011); 关注其不同的发展模式(朱旭 等, 2015)、人际互补发展趋势及其与咨询效果的关系(倪聪 等, 2020), 此外, 建立起良好的同盟也是临床心理咨询师的必备专长(Wampold & Imel, 2015)。尽管如此, 心理咨询中的同盟成分却依然难以理解和测量, 一方面, 同盟远远不是咨询师和来访者个人贡献的简单相加, 而是来自于双方的互动影响的过程, 关于同盟的核心理论应同样关注这种人际动力以及互动进程中的变化(Tschacher et al., 2015)。另一方面, 同盟的测量方法目前还多局限于来访者、咨询师或是第三方的主观自评, 自评方法局限于个体自身意识到的、能够报告、想要报告的部分, 在测量变化进程方面主要依赖于高级统计方法; 且自评大多聚焦于评估言语互动, 缺乏对其他非言语互动过程乃至神经活动的评估。
综上所述, 现代心理咨询领域对于心理咨询过程中的同盟研究虽然广泛且重要, 但在研究手段上仍有待于关注互动进程的测量方法, 在理论模型上有待于突破成分理论的局限, 从而才能真正揭示同盟发展的内在机制。
2 人际互动中的同步性现象及其测量
人际同步性的研究也得益于近年来测量方法的不断革新。具体而言, 在非言语同步(non-verbal synchrony)领域, 早期研究多为观察者对非言语同步进行主观评估, 通过对录像进行逐帧人工编码或请经过训练的研究人员对录像进行评分来评估人际互动过程中的动作同步。但是, 这种观察者评估的方法费时耗力且存在主观偏差。后期研究者继续发展了一些对人际同步进行自动量化评估的方法, 如框架差异法、时间线、交互迟滞回归以及动作能量分析(Ramseyer & Tschacher, 2016)等, 这些方法利用数字化的视频技术实现了对非言语同步的有效量化。例如, 动作能量分析(motion energy analysis, MEA; Ramseyer & Tschacher, 2011)是一种基于图像差分法这一计算机视觉领域的常用技术分析人际互动视频的方法, 通过逐帧计算像素灰度值的变化来量化动作幅度的大小。相对于主观评估而言, 动作能量分析更高效、客观(Ramseyer, 2020), 也为跨研究的比较提供了可能性。在将视频片段导入该程序后, 它会将导入的彩色视频转换为灰度视频并自动检测出每秒的帧数, 再逐帧计算帧与帧之间每个像素灰度值的变化量, 最后以兴趣区域(region of interest, ROI)内灰度值的总变化量作为该兴趣区的实时动作能量大小, 分析结束后会生成兴趣区域内动作能量时间序列的txt文件。但是, 动作能量分析的局限在于, 它只能量化动作的幅度, 却无法区分动作的方向和类型。
由于投入较低的测量成本就能获得高时间分辨率、客观且较为精确的数据, 生理同步近年来开始受到研究者的关注。目前研究最多的是自主神经系统(autonomic nervous system, ANS)的同步, 该系统与情绪反应相关, 且难以受到意识控制的影响。具体来说, 包括皮肤电同步(Chatel-Goldman et al., 2014)和心率同步(Ferrer & Helm, 2013; Montague et al., 2014)等。皮肤电同步基于对外泌汗腺活动性的测量, 与交感神经系统的活动相关; 心率同步常采用心电描记术和光学体积描记术进行数据采集, 与交感神经系统和非交感神经系统的活动均相关。此外, 研究者也开始将其他生理信号(如面部肌肉运动、皮肤温度、呼吸节律和谈话音高等)用于人际互动的设计中, 探讨这些生理信号与病人心理过程的关系(Palmieri, 2020)。然而, 由于不同的生理模态反映了不同的互动过程(Ferrer & Helm, 2013), 因此很难将不同模态下的生理同步进行比较, 结论的可推广性有限。
此外, 人际脑同步领域的超扫描研究(hyperscanning)使用脑电图(electroencephalography, EEG)、功能性磁共振成像(functional magnetic resonance imaging, fMRI)以及功能性近红外成像(functional near-infrared spectroscopy, fNIRS)等测量技术同时记录处于互动过程中的多个个体之间的大脑活动, 并考察分析个体大脑活动之间的关联。在测量原理上, fMRI技术和fNIRS技术通过血氧水平依赖(blood oxygenation level-dependent, BOLD)信号间接测量大脑活动, 而EEG技术则经由大脑皮质锥体细胞顶树突的突触后电位的变化直接测量大脑活动。虽然fMRI技术拥有高空间分辨率, 但无法创设一个真实的社会互动情境, 受试者不仅需要在整个实验过程中保持静止不动, 口头交流也因背景噪音的存在而受阻(Cui et al., 2012)。便携式EEG和fNIRS设备解决了上述问题, 提高了研究的生态效度, 测量成本相较于fMRI也更低, 因此被广泛应用于人际互动的研究中, 如教学情境下的师生互动(Bevilacqua et al., 2018; Dikker et al., 2017)、心理咨询中的咨访互动(Zhang et al., 2018; Zhang et al., 2020)、亲子互动(Reindl et al., 2018)、情侣互动(Long et al., 2021)、合作及竞争互动(Cui et al., 2012)。尤其是近年来发展起来的fNIRS技术, 相较于EEG技术的空间分辨率更高, 抗运动伪迹干扰的能力也更好(Reindl et al., 2018), 但fNIRS技术的局限在于仅能够测量大脑皮层的活动。
随着测量方法的不断推进, 研究者对人际同步性的研究更加充分, 并渗透到人际互动的各个领域。大量研究表明, 人际同步性不仅发展了个体的亲社会行为, 而且增进了互动双方良好的人际关系和共情性合作(Koehne et al., 2015), 可能是共情(Valdesolo & DeSteno, 2011)和成功合作(Valdesolo et al., 2010)的先决条件, 积极影响了个体内(如积极情绪、归属感)和人际间(如亲社会行为)的结果(Vicaria & Dickens, 2016)。以上人际同步性现象的研究提示, 采用不同测量方法评估的非言语同步、生理同步以及脑同步可能是良好的人际互动质量的指标, 有可能回应目前心理咨询领域中重要的同盟研究的局限, 即人际互动特点研究不足、测量方法局限于主观报告的困境。
3 理论争议:人际同步性现象与心理咨询中的同盟关系
大量研究发现良好的同盟能够促进治疗效果, 是跨越不同流派重要的疗效因子(Wampold & Imel, 2015)。关于同盟的研究目前主要使用主观报告, 该方法虽然简单、快速、经济且有良好的测量学特性, 但是局限于个体自身意识到的、能够报告、想要报告的部分(Flückiger et al., 2018), 无法回应同盟中的互动成分以及个体自身无法意识到的部分。近年来越来越多的研究聚焦于使用补充、额外的方法来评估同盟关系, 如使用操纵杆技术来实时评估流动的咨访行为(倪聪 等, 2020), 但依然采用观察者对录像进行主观报告评估。近年来随着测量人际同步性的技术革新, 该领域的研究初步揭示了不同情境下人际交流的互动特点及内在机制, 也为探索心理咨询领域的同盟提供了良好的技术支持和理论基础。但是, 不同模态的人际同步性现象是否能作为心理咨询同盟的评估指标, 是否能够彼此整合, 以揭示同盟建立过程中的人际互动特点还需要进一步细致的研究。以下将结合已有的研究进展和理论假设对该领域的研究进行梳理。
3.1 研究进展
具体来讲, 在心理咨询同盟领域的人际同步性研究主要包括非言语同步、生理同步、脑同步三个方面, 该领域的人际同步性研究一方面探索了其与同盟的关系, 另一方面还纳入了同盟的影响因素(如治疗师特点)及其对治疗效果的贡献, 初步揭示了人际同步性、同盟及其他因素之间的影响作用。
3.1.1 非言语同步(Non-Verbal Synchrony, NVS)
例如, 一些研究发现, 非言语同步与治疗效果及治疗中的共同因素(如治疗关系的质量和病人的自我效能感)存在显著相关(Koole & Tschacher, 2016; Tschacher & Pfammatter, 2016)。但是, 近期几项研究却发现, 非言语同步并非总是对同盟起到促进作用, 理想的非言语同步应为中等的同步性水平, 同步性过高会被认为是刻意模仿, 同步性过低又显得不够真诚(Moulder et al., 2018)。Paulick等人(2018)根据咨询效果将来访者分为改善组、无改善-双方自愿终止组及无改善-脱落组, 结果发现改善组表现出了中等水平的动作同步, 无改善-脱落组表现出了低水平的动作同步, 而无改善-双方自愿终止组表现出了高水平的动作同步。Ramseyer (2020)测量了12对咨询师和来访者在整个咨询片段中的非言语同步以及咨询师和来访者主观评估的同盟质量, 结果并未发现非言语同步和同盟之间存在此前研究中呈现的正相关。这些研究结果说明, 虽然非言语同步可能作为同盟的补充评估, 是一个具有前景的探索领域, 但结果还远远未达成一致。
基于元分析研究发现同盟与咨询效果之间存在较为稳定的相关(r = 0.28)且该效应量相当于Cohen' d = 0.58 (Flückiger et al., 2018), 不少研究者在探索非言语同步和同盟之间关系的同时, 也进一步研究了不同种类的非言语同步与咨询效果的关系。例如, Ramseyer和Tschacher (2011)根据互动过程中谁先发起动作, 将心理咨询中的非言语同步分为咨询师带领的同步和来访者带领的同步, 他们发现, 只有咨询师带领的非言语同步与同盟质量之间存在正相关, 而来访者带领的同步与来访者的自我效能感之间存在正相关。Altmann等人(2019)与Schoenherr等人(2019)均对社交焦虑障碍患者的第3次心理咨询过程有过研究, 前者发现, 由来访者带领的非言语同步与抑郁症状的严重程度呈正相关, 后者却发现, 由来访者带领的非言语同步与咨询脱落率之间呈负相关。
综上所述, 非言语同步虽然和咨询过程中的同盟具有千丝万缕的联系, 是一个具有前景的研究方向, 但是这两者之间的双向关系以及非言语同步是否能作为同盟质量的补充评估指标还需要进一步研究。针对目前不一致的研究结果, 一方面应该更为精细地区分同盟成分(如状态成分和特质成分), 另一方面需要对不同类型的非言语同步进行探索(如咨询师带领的和来访者带领的时间迟滞的同步), 以阐明两者之间多样化的关系。
3.1.2 生理同步(Physiological Synchrony, PS)
生理同步指多个个体在人际互动过程中所表现出的生理过程的协同现象(Mayo et al., 2021), 如心跳变得一致, 汗腺以同步的方式激活, 呼吸节律相互影响变得一致等。在临床领域之外的研究已经发现生理同步能够预测关系质量, 如支持性、同感能力以及适应性的情绪管理等(Palumbo et al., 2017), 而这些成分本身就是良好的同盟关系的核心。实际上, 上世纪50年代最早关于生理同步的三项研究就是关于心理咨询过程的, 该过程中的生理同步涉及皮肤电同步、心率、语速、沉默模式和语音韵律等各个方面, 近年来, 临床情境下的生理同步研究有所增加但还远远不足(Kleinbub, 2017)。
另一方面, 研究者还探索了生理同步与同盟之间紧密关系的促发原因, 其中一项重要发现是生理同步与治疗师的核心正性品质(如治疗师的共情力)相关。在高生理同步的咨询片段里存在更为积极的社会情绪互动, 且不同水平的临床训练与生理同步的显著性差异有关, 例如受过更多训练的心理咨询师与刚毕业的心理咨询师相比与来访者之间存在更高水平的3秒延迟的生理同步(即时间滞后的生理同步) (Kleinbub et al., 2020)。
综上所述, 心理咨询过程中咨访双方的生理同步可能是更为积极的同盟指标, 可用于客观评估咨询师的共情程度, 作为来访者口头报告(如口头报告被咨询师理解的程度)的重要补充。后续研究应继续关注如何促进咨询师和来访者之间发生生理同步, 并综合考虑生理同步和非言语同步的关系。
3.1.3 人际脑同步(Interpersonal Brain Synchrony, IBS)
人际脑同步是指互动个体之间大脑活动的同步性(即脑间同步), 具体指在脑信号的激活水平、成分、时间、空间、频率、相位等维度上计算个体间大脑活动的变化关联性(李先春, 2018)。在过去的10年里, 大量研究发现人际脑同步水平与互动双方良好的沟通分享、心理协调有关(Mu et al., 2018); 这种同步基于互动双方的眼神凝视和正性情感, 与其社会联结的水平呈正相关(Kinreich et al., 2017)。实际上, 近期研究者发现在合作和人际互动的过程中存在人际脑同步增强的现象, 这也说明脑同步可能是良好人际沟通的脑基础; 此外, 在言语沟通(Jiang et al., 2015)、非言语沟通(Ramseyer & Tschacher, 2011)和半言语沟通(Osaka et al., 2015)中都发现了脑同步对良好沟通的贡献。
另有研究者关注非言语同步和脑同步之间的紧密联系。例如, 在运动相关的任务中, 研究者发现运动同步和脑间联结之间存在显著相关; 当参与谈话的互动双方面对面时, 其脑同步比背对背时更强, 这可能是因为面对面的互动使得动作同步的发生成为可能(Jiang et al., 2012)。虽然根据以上研究可以初步推测, 非言语同步可能促进了脑同步的发生, 但在心理咨询情境下两者之间是否存在这种促进关系, 依然缺乏实证研究。
近期一项研究使用近红外超扫描技术比较了心理咨询和闲聊过程中的人际脑同步, 结果发现常规心理咨询过程中的人际脑同步显著高于闲聊过程中的脑同步, 且这种脑同步发生在与心理理论、认知共情密切相关的右侧颞顶联合区(Zhang et al., 2018)。该团队的后续研究进一步对有经验的咨询师在首次心理咨询过程中的人际脑同步情况进行研究, 并与新手咨询师的首次工作过程进行比较, 结果发现有经验的咨询师在和来访者工作过程中与新手咨询师相比出现了更强的脑同步以及来访者引导的时间迟滞的脑同步(Zhang et al., 2020)。这两项研究一定程度上揭示了人际脑同步与心理咨询同盟之间存在密切关系, 人际脑同步有可能作为心理咨询同盟建立的神经指标, 作为评估同盟质量的补充, 但是关于脑同步、非言语同步、生理同步及咨询效果的研究还有待于进一步探索。
3.2 理论假设与争议
实际上, 研究者对于心理咨询情境下的非言语同步、生理同步以及脑同步如何用于评估同盟依然存在争议, 基于同步性与同盟之间作用机制的理论模型也正在发展中。例如, 在不同模态的人际同步和同盟的关系方面, 一些研究者认为包括表情同步、生理同步等在内的测量结果可能是评估同盟的补充方式, 尤其可用于评估同盟中的互动成分, 如非言语同步可作为评估对抗性同盟破裂的标记(Cohen et al., 2021), 或是评估心理咨询情境下的非言语互动(Tschacher & Meier, 2020)。实际上, 非言语互动是咨询中相当重要却较少被关注和研究的部分(Paulick et al., 2018), 以下理论从各个角度揭示了不同模态的人际同步性促进非言语互动和同盟背后可能的内在机制。
一个和非言语同步相关的成分是咨访双方的目标(意向)达成一致, 研究者提出了共享意向理论来诠释意向一致性对合作行为的影响。共享意向是指两个或两个以上的个体通过协调自己的行为来实现共同的目标。在社会互动的研究中已经发现, 人类倾向于无意识地将自己的行为动作与共同从事某项任务的人相互协调, 这种行为上的协调同步会促进同伴间的互动, 加深彼此的喜爱。基于共享意向的人际同步能够更大程度地促进合作行为。此外, 人际脑同步可以预测人际间的亲社会倾向, 并且共享意向在人际脑同步和亲社会倾向之间起中介作用(Hu et al., 2017)。共享意向可能是非言语同步产生一系列积极的社会心理效应的内在机制。
实际上, 已有的理论假设更多关注生理同步如何促进心理咨询过程中的同盟, 一个可能的解释是迷走神经学说, 该学说认为治疗师和病人之间协调的生理反应可能是安全感的生理行为基础, 使得治疗关系得以发展(Geller & Porges, 2014)。另一个可能的理论假设是社会心理理论, 即治疗师作为一名照顾者的角色在生理上努力与来访者形成协调一致, 就好像早期养育者帮助小婴儿管理他们的内在生理和行为一样(Gilbert, 2019)。还有一些研究者提出了有关生理同步促进咨询效果的理论假设, 如Feldman (2020)提出的心理弹性模型将人际同步性视为建立个体心理弹性的核心。具体来说, 人际同步性促使自主的自我和他人融合, 即在一个安全的对话框架下, 将人际互动中匹配的瞬间与不匹配的瞬间、独自的状态以及修复的片段整合起来。在大量跨越不同文化、跨越人类发展阶段(从婴儿期到成年期)以及从健康人群到高风险人群的研究中, 均发现了这种珍贵的“社交时刻”, 即父母和孩子自发调整他们的非言语行为以促进互动双方在生物学上的同步协调。只有在这样的时刻, 母亲和婴儿的心跳同步, 且大脑出现同步(Levy et al., 2017), 这种同步性与更好的压力管理能力和更健康的免疫系统功能相关(Ulmer-Yaniv et al., 2018)。具体来说, 协调的社会行为能够降低个体的压力并促进其抗逆力的提升, 而该过程很有可能同样发生在相互深入交流的心理咨询过程中, 即人际同步性可能促进了来访者功能的改善以及咨询效果的发生。
此外, Zilcha-Mano等人(2021)近期提出催产素同步(oxytocin synchrony)可能是同盟建立的内在生物学机制。已有的研究发现催产素系统在人际互动和同步性行为中发挥着重要的作用(如母婴关系、浪漫关系等)。在心理治疗过程中, 催产素同步是指治疗师和来访者在咨询前后催产素改变程度之间的相关, 如果他们的催产素水平表现出相同方向的改变(如两者都提高), 则视为产生了催产素同步(Zilcha-Mano et al., 2020)。在咨询过程中, 当治疗师和来访者在催产素水平的改变上产生生物学同步时, 带来了协调有效的关系互动, 治疗才是有效的, 这种催产素系统的功能提升可能是依恋或同盟促进脑同步的神经生物学机制。
已有的人际脑同步研究虽然初步表明咨访脑同步和同盟质量存在正相关, 但是, 对该领域的探索才刚刚起步, 缺乏对不同模态的人际同步和同盟进行综合考虑的整合研究。不过, 近期也有研究者已经提出了关于心理咨询中同盟建立的理论模型, 综合考虑了不同模态的人际同步, 对咨访同盟建立的内在机制进行了假设。例如, Koole和Tschacher (2016)提出了人际同步模型(Interpersonal Synchrony Model, In-Sync Model), 该模型认为同盟来源于治疗师和来访者之间大脑神经活动的匹配, 咨访之间的大脑活动越匹配, 同盟越好。这种同步性是同盟质量的具身表现, 进而通过同盟的质量调节咨询效果。根据该模型, 在心理咨询过程中, 基于感知运动过程(perceptual-motor processes)的动作同步是最基本的人际同步形式, 无需意识参与就能实现; 在动作同步的基础上, 咨访双方形成了基于社会认知过程(social-cognitive processes)的人际脑同步, 使得咨询师和来访者能够进入另一个人的内在世界, 促进了彼此的理解和情绪分享, 是同盟形成的关键; 而同盟包括共同语言、自我的分享以及情绪相互管理, 通过提升来访者的情绪调节能力促进了治疗效果的发生。
以上理论解释关注不同模态的人际同步促进同盟建立的内在机制, 如共享意向理论假设互动双方通过行为协调促进共享意向进而实现共同目标, 达成同盟; 迷走神经学说、社会心理理论以及心理弹性模型从不同角度诠释了人际同步促进同盟乃至咨询效果的内在原理; 催产素同步假说关注同盟建立的内在生物学机制, 而人际同步模型整合了这些不同模态的同步对同盟建立的内在运作过程进行了理论假设。但是, 这些理论模型依然需要实验研究的进一步验证与完善, 不同模态同步性之间的相互关联目前还处于理论假设的阶段。
4 人际同步现象在心理咨询领域的研究展望
整合取向的心理治疗倾向于将心理治疗视为一个动力系统, 该系统包括病人和治疗师以及他们各自的子系统(如心率血压、神经系统等), 在病人和治疗师互动时这些子系统也在相互影响, 从这个角度来说, 这些子系统如何协同合作对于有效的心理治疗尤为重要。心理咨询过程中人际同步性现象的研究进展有助于揭示该动力系统的交互作用过程, 并为评估同盟提供可能的补充。具体来说, 人际同步性现象在心理咨询领域未来的研究包括以下三点:
4.1 人际同步现象可能揭示心理咨询过程中同盟建立的内在机制
关于咨访同盟的当代临床研究虽然已经取得了重要的理论和实践研究进展, 但是, 关于同盟建立的人际互动特点却依然不甚清楚, 且评估方法大多局限于主观报告。实际上, 同盟并不是咨询师和来访者对于关系的贡献之和, 而是出现在他们的互动中, 进而产生相互影响, 是一种相互关联的现象。任何时候当人们互动时都倾向于自发地同步他们的神经、知觉、情感、生理以及行为反应(Repp & Su, 2013), 这种人际同步现象可能是同盟建立的内在机制, 对于咨询过程中非言语同步、生理同步、脑同步乃至生物学同步的研究有望揭示同盟中的人际动力本质(Koole et al., 2020)。此外, 已有的研究主要关注单次咨询过程中的同盟及其人际同步现象, 未来研究还需注意到同盟的动态发展特点, 关注同盟动态变化过程中的不同模态的人际同步变化, 采用重复测量, 用各种不同的模型考察多方评估的同步性、一致性问题和变化特点, 为临床工作提供参考。
4.2 发展能够引发多模态人际同步性的心理咨询, 促进临床疗效
已有的社会心理研究已经发现, 引导人们以同步的方式运动能够促进合作和互助行为, 促进人们彼此之间的喜欢、同情、亲密以及互动双方之间反应敏感性的增加。人际同步可能是提高社会协调性的内在机制。实际上, 如何更有效地与来访者形成同步是咨询师培训中重要的非言语技能训练。目前, 如何在技术层面促进咨询师和来访者的人际同步的研究依旧不足, 这种同步性是否能带来良好的咨询效果也正在研究中(Mende & Schmidt, 2021)。例如, 在对于社交焦虑者的临床干预中, 研究者发现更好的非言语同步(第三次和第八次咨询时)能够预测结束治疗时个案在人际困扰方面更低的得分(Altmann et al., 2019)。未来对于咨访同步性的研究可能促进咨询师根据来访者的特点更好地与之形成非言语同步以及脑同步, 进而推动良好的同盟建立, 发展更有效的心理咨询服务。
4.3 整合不同模态人际同步与同盟关系, 发展研究证据支持的理论假说
尽管已有的理论模型关注人际同步促进同盟背后可能的内在机制, 提出了其可能的生物学基础(如催产素同步)、神经学说(如迷走神经理论)或提出了初步的理论模型(心理治疗的人际同步模型)来诠释不同模态的同步如何促进了同盟的建立, 但是这些理论假说还需要大量的研究证据支持。即便是整合不同模态同步性的In-Sync模型(Koole & Tschacher, 2016)依然存在明显不足, 如未考虑来访者期待因素对同盟的影响, 也同样缺乏研究证据的支持; 未能区分同盟的特质成分和状态成分, 且不同模态人际同步之间的关系以及和同盟质量之间的关系还需要进一步研究证据, 未来研究可以同时测量不同模态的人际同步和同盟, 同时考虑来访者因素的影响, 进一步完善理论模型。
5 总结
近10年以来, 人际同步性的研究取得了重要的研究进展和技术突破, 为揭示心理咨询过程中咨访同盟的互动特点和内在机制提供了可能性。本文梳理了不同模态的人际同步性与同盟之间的关系及已有理论如何阐释其内在机制, 并对未来的研究进行了展望。具体来说, 包括以下5个要点: (1)咨访双方的生理同步是更为积极的咨访同盟的指标, 或可用于客观评估咨询师的共情程度; (2)非言语同步与同盟之间的双向关系以及非言语同步是否能作为同盟质量的补充评估指标还需要进一步研究, 应更为精细地区分同盟成分和不同类型的非言语同步; (3)人际脑同步有可能作为咨访同盟的神经指标, 作为评估同盟质量的补充; (4)在同盟建立的内在机制方面, 已有的理论包括共享意向理论、迷走神经学说、社会心理理论以及心理弹性模型等, 从不同角度诠释了人际同步促进同盟乃至咨询效果的内在原理; 此外, 催产素同步假说关注同盟建立的内在生物学机制; 人际同步模型整合了这些不同模态的同步对同盟建立的内在运作过程进行了理论假设。(5)未来的研究者可以更多地关注同盟动态变化过程中的人际同步性、发展整合理论来回应同盟建立的内在机制, 并在临床应用上促进具有咨访同步性的心理咨询互动过程。

Interpersonal touch is of paramount importance in human social bonding and close relationships, allowing a unique channel for affect communication. So far the effect of touch on human physiology has been studied at an individual level. The present study aims at extending the study of affective touch from isolated individuals to truly interacting dyads. We have designed an ecological paradigm where romantic partners interact only via touch and we manipulate their empathic states. Simultaneously, we collected their autonomic activity (skin conductance, pulse, respiration). Fourteen couples participated to the experiment. We found that interpersonal touch increased coupling of electrodermal activity between the interacting partners, regardless the intensity and valence of the emotion felt. In addition, physical touch induced strong and reliable changes in physiological states within individuals. These results support an instrumental role of interpersonal touch for affective support in close relationships. Furthermore, they suggest that touch alone allows the emergence of a somatovisceral resonance between interacting individuals, which in turn is likely to form the prerequisites for emotional contagion and empathy.

We used Near-Infrared Spectroscopy (NIRS) to simultaneously measure brain activity in two people while they played a computer-based cooperation game side by side. Inter-brain activity coherence was calculated between the two participants. We found that the coherence between signals generated by participants' right superior frontal cortices increased during cooperation, but not during competition. Increased coherence was also associated with better cooperation performance. To our knowledge, this work represents the first use of a single NIRS instrument for simultaneous measurements of brain activity in two people. This article reports on a research synthesis of the relation between alliance and the outcomes of individual psychotherapy. Included were over 200 research reports based on 190 independent data sources, covering more than 14,000 treatments. Research involving 5 or more adult participants receiving genuine (as opposed to analogue) treatments, where the author(s) referred to one of the independent variables as "alliance," "therapeutic alliance," "helping alliance," or "working alliance" were the inclusion criteria. All analyses were done using the assumptions of a random model. The overall aggregate relation between the alliance and treatment outcome (adjusted for sample size and non independence of outcome measures) was r =.275 (k = 190); the 95% confidence interval for this value was.25-.30. The statistical probability associated with the aggregated relation between alliance and outcome is p <.0001. The data collected for this meta-analysis were quite variable (heterogeneous). The neural mechanism of leader emergence is not well understood. This study investigated (i) whether interpersonal neural synchronization (INS) plays an important role in leader emergence, and (ii) whether INS and leader emergence are associated with the frequency or the quality of communications. Eleven three-member groups were asked to perform a leaderless group discussion (LGD) task, and their brain activities were recorded via functional near infrared spectroscopy (fNIRS)-based hyperscanning. Video recordings of the discussions were coded for leadership and communication. Results showed that the INS for the leader-follower (LF) pairs was higher than that for the follower-follower (FF) pairs in the left temporo-parietal junction (TPJ), an area important for social mentalizing. Although communication frequency was higher for the LF pairs than for the FF pairs, the frequency of leader-initiated and follower-initiated communication did not differ significantly. Moreover, INS for the LF pairs was significantly higher during leader-initiated communication than during follower-initiated communications. In addition, INS for the LF pairs during leader-initiated communication was significantly correlated with the leaders' communication skills and competence, but not their communication frequency. Finally, leadership could be successfully predicted based on INS as well as communication frequency early during the LGD (before half a minute into the task). In sum, this study found that leader emergence was characterized by high-level neural synchronization between the leader and followers and that the quality, rather than the frequency, of communications was associated with synchronization. These results suggest that leaders emerge because they are able to say the right things at the right time.

The aim of this study was to test whether there is statistically significant sympathetic nervous system (SNS) synchrony between participants in couple therapy. To our knowledge, this is the first study to measure psychophysiological synchrony during therapy in a multiactor setting. The study focuses on electrodermal activity (EDA) in the second couple therapy session from 10 different cases (20 clients, 10 therapists working in pairs). The EDA concordance index was used as a measure of SNS synchrony between dyads, and synchrony was found in 85% of all the dyads. Surprisingly, co-therapists exhibited the highest levels of synchrony, whereas couples exhibited the lowest synchrony. The client-therapist synchrony was lower than that of the co-therapists, but higher than that of the couples. The evolution of humans as a highly social species tuned the brain to the social world; yet the mechanisms by which humans coordinate their brain response online during social interactions remain unclear. Using hyperscanning EEG recordings, we measured brain-to-brain synchrony in 104 adults during a male-female naturalistic social interaction, comparing romantic couples and strangers. Neural synchrony was found for couples, but not for strangers, localized to temporal-parietal structures and expressed in gamma rhythms. Brain coordination was not found during a three-minute rest, pinpointing neural synchrony to social interactions among affiliative partners. Brain-to-brain synchrony was linked with behavioral synchrony. Among couples, neural synchrony was anchored in moments of social gaze and positive affect, whereas among strangers, longer durations of social gaze and positive affect correlated with greater neural synchrony. Brain-to-brain synchrony was unrelated to episodes of speech/no-speech or general content of conversation. Our findings link brain-to-brain synchrony to the degree of social connectedness among interacting partners, ground neural synchrony in key nonverbal social behaviors, and highlight the role of human attachment in providing a template for two-brain coordination.

Introduction: The fast expanding field of Interpersonal Physiology (IP) focuses on the study of co-ordination or synchronization dynamics between the physiological activities of two, or more, individuals. IP has been associated with various relational features (e.g., empathy, attachment security, rapport, closeness.) that overlap with desirable characteristics of clinical relationships, suggesting that the relevant studies might provide objective, economical, and theory-free techniques to investigate the clinical process. The goal of the present work is to systematically retrieve and review the literature on IP in the field of psychotherapy and psychological intervention, in order to consolidate the knowledge of this research domain, highlight its critical issues, and delineate possible developments.Method: Following the guidelines by Okoli and Schabram (2010), a systematic literature search was performed in Scopus, Web of Science, PsycINFO, and PubMed databases by means of multiple keyword combinations; the results were integrated with references to the retrieved articles' bibliography as well as to other published reviews on IP.Results: All the retrieved documents reported clinical interactions that are characterized, at least partially, by IP phenomena. They appear to use fragmented and sometimes ambiguous terminology and show a lack of both specific theory-informed hypotheses and sound analytical procedures.Conclusion: Although the psychological nature of IP and its role in the clinical relationship are still mostly unknown, the potential value of a physiology-based measure of implicit exchanges in psychotherapy drives an acceleration in this research field. On the basis of the highlighted critical issues, possible future directions for clinical IP researchers are discussed.

Physiological synchronization is the study of how individuals in interaction coregulate their physiology. The topic has sparked increasing interest in counseling and psychotherapy research, where it has been found to be associated with the therapeutic alliance, clinicians' empathy and patients' outcome. Physiological synchronization allows researcher to investigate subtle but fundamental aspects of the clinical process through objective measures. In this article, we aim to offer a guide to researchers and clinicians to explore this growing field of study. We begin by reviewing the existing literature of physiological synchronization in clinical relationships, and then we provide practical guidelines for research. We discuss the various aspects involved in synchronization studies: study design, selection of physiological signals, data analytic approaches, and interpretation of results. To better illustrate how to implement these types of design, we provide a running example describing the data collection and analysis of a single-case study. In the example we discuss both how to conduct a longitudinal nomothetic analysis, as well as a moment-to-moment idiographic exploration of the clinical content. In this latter analysis, in particular, we show how physiological synchronization can be used in combination with 2 transcripts analysis tools, the Patient Attachment Coding System, and the Therapist Attunement Scales to reach a deeper understanding of the ongoing processes. We conclude by arguing that research in counseling and psychotherapy has much to gain from and contribute to the overall development of our understanding of physiological synchronization in human interaction. The quality of a team depends on its ability to deliver information through a hierarchy of team members and negotiate processes spanning different time scales. That structure and the behavior that results from it pose problems for researchers because multiply-nested interactions are not easily separated. We explored the behavior of a six-person team engaged in a Submarine Piloting and Navigation (SPAN) task using the tools of dynamical systems. The data were a single entropy time series that showed the distribution of activity across six team members, as recorded by nine-channel electroencephalography (EEG). A single team's data were analyzed for the purposes of illustrating the utility of multifractal analysis and allowing for in-depth exploratory analysis of temporal characteristics. Could the meaningful events experienced by one of these teams be captured using multifractal analysis, a dynamical systems tool that is specifically designed to extract patterns across levels of analysis? Results indicate that nested patterns of team activity can be identified from neural data streams, including both routine and novel events. The novelty of this tool is the ability to identify social patterns from the brain activity of individuals in the social interaction. Implications for application and future directions of this research are discussed.

Synchrony between interacting systems is an important area of nonlinear dynamics in physical systems. Recently psychological researchers from multiple areas of psychology have become interested in nonverbal synchrony (i.e., coordinated motion between two individuals engaged in dyadic information exchange such as communication or dance) as a predictor and outcome of psychological processes. An important step in studying nonverbal synchrony is systematically and validly differentiating synchronous systems from nonsynchronous systems. However, many current methods of testing and quantifying nonverbal synchrony will show some level of observed synchrony even when research participants have not interacted with one another. In this article we demonstrate the use of surrogate data generation methodology as a means of testing new null-hypotheses for synchrony between bivariate time series such as those derived from modern motion tracking methods. Hypotheses generated by surrogate data generation methods are more nuanced and meaningful than hypotheses from standard null-hypothesis testing. We review four surrogate data generation methods for testing for significant nonverbal synchrony within a windowed cross-correlation (WCC) framework. We also interpret the null-hypotheses generated by these surrogate data generation methods with respect to nonverbal synchrony as a specific use of surrogate data generation, which can then be generalized for hypothesis testing of other psychological time series. A methodological obstacle for the assessment of psychotherapy process concerns the compatibility between nomothetic versus idiographic approaches. Using quantitative methodology at an idiographic level, we seek to narrow this gap for the special case of nonverbal synchrony in psychotherapy dyads. Using quantitative idiographic process analysis (QUIPA), we combined the assessments of nonverbal synchrony-the coordination of patient's and therapist's body-movement-quantified with motion energy analysis (MEA), patient- and therapist session-reports, and patient's self-reports of symptomatology into multivariate time-series of psychotherapy process across the course of = 12 dyadic psychotherapies with a total of = 150 sessions. Adopting both mixed models as well as vector auto regressive models (VAR) to time-series, we aimed to extend previous results assessed in a randomized sample by exploring the evolution of nonverbal synchrony in psychotherapy dyads across time. Aggregated idiographic data revealed that nonverbal synchrony was different from findings based on a previously reported nomothetic account: Associations with session-level outcome were weaker and did not replicate cross-sectional analyses. The application of idiographic assessments may help broadening our conception of synchrony in psychotherapy and also reminds us that ergodicity is not necessarily the usual case, but one of many possibilities.

The authors quantified nonverbal synchrony--the coordination of patient's and therapist's movement--in a random sample of same-sex psychotherapy dyads. The authors contrasted nonverbal synchrony in these dyads with a control condition and assessed its association with session-level and overall psychotherapy outcome.Using an automated objective video analysis algorithm (Motion Energy Analysis; MEA), the authors calculated nonverbal synchrony in (n = 104) videotaped psychotherapy sessions from 70 Caucasian patients (37 women, 33 men, mean age = 36.5 years, SD = 10.2) treated at an outpatient psychotherapy clinic. The sample was randomly drawn from an archive (N = 301) of routinely videotaped psychotherapies. Patients and their therapists assessed session impact with self-report post-session questionnaires. A battery of pre- and postsymptomatology questionnaires measured therapy effectiveness.The authors found that nonverbal synchrony is higher in genuine interactions contrasted with pseudointeractions (a control condition generated by a specifically designed shuffling procedure). Movement coordination in psychotherapy: Synchrony of hand movements is associated with session outcome. A single-case study
Previous work has shown that nonverbal behavior was associated with both session-level outcome and global outcome in psychotherapy. Nonverbal synchrony--here the coordination between patient's and psychotherapist's movement behavior--is a facet of nonverbal behavior that has recently been studied with video-based motion energy analysis (MEA). The present study aimed to replicate and extend these findings by using direct acquisition of movement data. In a single-case analysis, we monitored patient's and therapist's hand movements with a high-resolution accelerometric measurement system (Vitaport (r)). In addition to these behavioral data, both patient and therapist provided session-level ratings of various factors relevant to the psychotherapy process, which were assessed with post-session questionnaires. The patient-therapist coordination of hand movements, i.e. nonverbal synchrony, in (N = 27) sessions of this dyadic psychotherapy was positively associated with progress reported in post-session questionnaires. Sessions with good evaluations concerning the quality of therapeutic alliance were characterized by high movement coordination. Thus, accelerometric data of this therapy dyad confirmed previous findings gained through video analyses: The coordination of nonverbal behavior shown by patient and therapist was an indicator of beneficial processes occurring within sessions. This replication study showed that nonverbal synchrony embodies important aspects of the alliance. Its assessment and quantification may provide therapists important additional information on processes that usually occur outside conscious awareness, but that nevertheless influence core aspects of the therapy.

The capacity to establish interpersonal synchrony is fundamental to human beings because it constitutes the basis for social connection and understanding. Interpersonal synchrony refers to instances when the movements or sensations of two or more people overlap in time and form. Recently, the causal influence of interpersonal synchrony on prosociality has been established through experiments. The current meta-analysis is the first to synthesize these isolated and sometimes contradictory experiments. We meta-analyzed 60 published and unpublished experiments that compared an interpersonal synchrony condition with at least one control condition. The results reveal a medium effect of interpersonal synchrony on prosociality with regard to both attitudes and behaviors. Furthermore, experimenter effects and intentionality moderate these effects. We discuss the strengths and limitations of our analysis, as well as its practical implications, and we suggest avenues for future research.

Premature termination is a problem in psychotherapy. In addition to the examination of demographic and clinical variables as predictors of dropout, research indicates the importance of dyadic variables. Nonverbal synchrony (e.g., movement synchrony) operationalizes the coordination of patient and therapist and is a promising candidate for predicting premature termination. This secondary data analysis included data on patients with social anxiety disorder ( = 267) that were treated with > 20 sessions of cognitive-behavioral therapy or psychodynamic therapy. Therapy outcome was measured by the Inventory of Interpersonal Problems and the Beck Depression Inventory. Individual movements in the third session were assessed by motion energy analysis. Movement synchrony was identified with a windowed cross-lagged correlation and peak-picking algorithm. We performed logistic regressions and mixed effects Cox regressions to investigate synchrony as a predictor of premature termination. Therapist-patient dyads that included a patient who terminated psychotherapy prematurely had significantly lower movement synchrony at the beginning of therapy than patients who completed therapy. Gender-matching and therapeutic approach had a (marginally) significant effect in moderating the relationship. Therefore, low movement synchrony in early therapy sessions may contain clues to premature termination and reflect a mismatch between the patient and therapist or problems in the therapeutic alliance. Sympathetic nervous system synchrony: An exploratory study of its relationship with the therapeutic alliance and outcome in couple therapy

In previous research, we found that sympathetic nervous system synchrony, measured via electrodermal activity (EDA), occurs between participants at the start of couple therapy. The aim now was to test whether this synchrony changes during the therapy process, and how any changes may be related to clients' and therapists' evaluations of the working alliance, and the outcome of therapy. Twelve different couple therapy processes were analyzed (24 clients, plus 10 therapists, working in pairs; hence, 4 persons per session) using EDA concordance indices and questionnaires (Outcome Rating Scale, Session Rating Scale, and Clinical Outcomes in Routine Evaluation-Outcome Measure). EDA synchrony between the couples increased from the beginning to the end of therapy. This seemed to be connected to a positive linear trend in female clients' well-being during the therapy process. There were no statistically significant changes in the EDA synchrony between the cotherapists, or between the clients and the therapists. We found specific changes in the EDA synchrony to be related to changes in the therapeutic alliance, and/or changes in the clients' well-being. Heightened EDA synchrony was frequently related to a better outcome; nevertheless, there was one instance in which decreasing synchrony seemed to be more beneficial. It appears that couple therapy can bring spouses closer together also on a physiological level, which could be especially important for the well-being of women. There is broad consensus that the therapeutic alliance constitutes a core common factor for all modalities of psychotherapy. Meta-analyses corroborated that alliance, as it emerges from therapeutic process, is a significant predictor of therapy outcome. Psychotherapy process is traditionally described and explored using two categorically different approaches, the experiential (first-person) perspective and the behavioral (third-person) perspective. We propose to add to this duality a third, structural approach. Dynamical systems theory and synergetics on the one hand and enactivist theory on the other together can provide this structural approach, which contributes in specific ways to a clarification of the alliance factor. Systems theory offers concepts and tools for the modeling of the individual self and, building on this, of alliance processes. In the enactive perspective, the self is conceived as a socially enacted autonomous system that strives to maintain identity by observing a two-fold goal: to exist as an individual self in its own right (distinction) while also being open to others (participation). Using this conceptualization, we formalized the therapeutic alliance as a phase space whose potential minima (attractors) can be shifted by the therapist to approximate therapy goals. This mathematical formalization is derived from probability theory and synergetics. We draw the conclusion that structural theory provides powerful tools for the modeling of how therapeutic change is staged by the formation, utilization, and dissolution of the therapeutic alliance. In addition, we point out novel testable hypotheses and future applications.

In this proof-of-principle study, a convenience sample of 55 dyadic psychotherapy sessions conducted by one therapist was analyzed. This study aimed at exploring physiological synchrony in naturalistic psychotherapy sessions and the association of such synchrony with self-report ratings. The electrocardiograms and respiration behavior of both therapist and client were monitored simultaneously. Four clients were included, and session outcome was documented by session reports in two clients. From electrocardiograms, heart rate and heart rate variability were derived in consecutive 15-second intervals throughout sessions. Entire sessions (average duration, 51 min) were assessed for physiological synchrony of therapist's and client's respiration, electrocardiogram, heart rate, and heart rate variability. Two methods of synchrony computation were applied to the time series: windowed cross-correlation and correlation of local slopes (concordance). Both methods included surrogate controls using segment-wise shuffling. Significant synchrony of three measures, but not of electrocardiograms, was present in this dataset. In regression models, we found associations between synchronies and alliance ratings, and further self-report variables. Results support the existence of physiological synchrony in this collection of psychotherapy sessions, which speaks for the sympathetic and parasympathetic coupling between this therapist and her clients and its link with ratings of the therapy process. The feasibility of deriving signatures of synchrony of physiological signals with the described methodology was corroborated. The findings now await generalization by further research.

Chronic early trauma alters children's stress reactivity and increases the prevalence of anxiety disorders; yet the neuroendocrine and immune mechanisms underpinning this effect are not fully clear. Animal studies indicate that the mother's physiology and behavior mediate offspring stress in a system-specific manner, but few studies tested this external-regulatory maternal role in human children exposed to chronic stress.We followed a unique cohort of children exposed to continuous wartime trauma (N = 177; exposed; N = 101, controls; N = 76). At 10 years, maternal and child's salivary immunoglobulin A (s-IgA) and oxytocin (OT), biomarkers of the immune and affiliation systems, were assayed, maternal and child relational behaviors observed, mother and child underwent psychiatric diagnosis, and child anxiety symptoms assessed.War-exposed mothers had higher s-IgA, lower OT, more anxiety symptoms, and their parenting was characterized by reduced sensitivity. Exposed children showed higher s-IgA, more anxiety disorders and post traumatic stress disorder, and more anxiety symptoms. Path analysis model defined three pathways by which maternal physiology and behavior impacted child anxiety; (a) increasing maternal s-IgA, which led to increased child s-IgA, augmenting child anxiety; (b) reducing maternal OT, which linked with diminished child OT and social repertoire; and (c) increasing maternal anxiety, which directly impacted child anxiety.Our findings, the first to measure immune and affiliation biomarkers in mothers and children, detail their unique and joint effects on children's anxiety in response to stress; highlight the relations between chronic stress, immune activation, and anxiety in children; and describe how processes of biobehavioral synchrony shape children's long-term adaptation.

Oxytocin (OT) synchrony has been suggested as a key mechanism by which bonds are formed and strengthened in various species, including those between mother and infant and between romantic partners. It is unknown whether such biological synchrony also plays a role in psychotherapy efficacy, where it may underlie the adverse effect of social impairment on the efficacy of treatment of depression.Five hundred eighty OT saliva samples were collected from 37 patient-therapist dyads on a fixed schedule over a 16-session ongoing randomized controlled trial for psychotherapy for depression. Biological synchrony was operationalized as the correlation between changes occurring repeatedly over treatment in patient and therapist OT levels pre- to postsession.OT synchrony between patients and therapists was found to be associated with effective treatment. The findings support the proposed mediation model: (a) poorer social functioning at baseline predicted lower levels of patient-therapist synchrony in OT changes from pre- to postsession over the course of treatment; (b) lower levels of therapist-patient OT synchrony, in turn, predicted less reduction in depressive symptoms during treatment; and (c) based on quasi-Bayesian Monte Carlo simulations, the levels of therapist-patient synchrony significantly mediated the association between social impairment and reduction in depressive symptoms. Findings were replicated using robust inferential methods.The findings suggest that OT synchrony between patient and therapist may be a biological mechanism by which impaired interpersonal functioning undermines treatment outcome. The therapeutic alliance is one of the most consistent predictors of therapeutic change, including symptom reduction and improvement in wellbeing and quality of life, across a variety of mental health interventions. Yet, little is known about its biological mechanisms. Oxytocin (OT) has been suggested as a biological mechanism by which bonds are formed and strengthened across species. This article is intended to demonstrate the potential of OT as a biomarker of therapeutic change in psychotherapy and counseling psychology, especially of the therapeutic alliance. We delineate three main potential paths of investigation based on the most recent research on OT in parent-child and romantic partner dyads. For each path, we provide a detailed explanation for whom, when, and how OT should be measured. Each path is illustrated using data collected in a randomized controlled trial of psychotherapy for major depressive disorder. These illustrations demonstrate the great potential of OT as a biomarker of (a) trait-like characteristics of the patients and the therapists, (b) the processes of therapeutic change, and (c) the dyadic synchrony between patients and their therapists. The potential clinical contribution of OT as a biomarker for each of these three paths is further demonstrated using a case study.
